CVS Raises Full-Year Outlook After Benefits Profit Recovers

Third-quarter revenue reached $108.4 billion and the company lifted 2026 adjusted EPS guidance to at least $8.20 a share.

CVS Health Corporation (CVS) raised its full-year 2026 outlook after Health Care Benefits adjusted operating income more than doubled from a year earlier and the company generated $15.3 billion of cash from operations in the nine months ended September 30, 2026.

The health solutions company now projects GAAP diluted earnings of at least $6.98 a share, up from a range of $6.84 to $7.04, and adjusted earnings of at least $8.20 a share, up from $7.90 to $8.10. Cash flow from operations guidance moved to at least $15.5 billion from at least $11.5 billion. The company said the increases reflect the Health Care Benefits and Pharmacy & Consumer Wellness segments, while it maintains a cautious view for the rest of the year given elevated cost trends and potential macro headwinds.

For the three months ended September 30, 2026, total revenues were $108.4 billion, up 5.4% from $102.9 billion a year earlier. That year-over-year growth rate slowed from 7.3% in the second quarter. GAAP diluted earnings were $1.68 a share versus a $3.13 loss a year earlier, which the company said reflected the absence of a $5.7 billion goodwill impairment charge recorded in the third quarter of 2025. Operating income was $3.6 billion versus an operating loss of $3.2 billion. Adjusted operating income was $4.19 billion, up 21.2%.

Adjusted EPS was $2.09 a share versus $1.60 a year earlier. The company said the increase was primarily due to improved adjusted operating income in Health Care Benefits, reflecting continued execution of its margin recovery plan.

Health Care Benefits revenue was $36.9 billion, up 2.4%, driven by the Government business and partially offset by the company’s 2026 exit of the individual exchange business. Adjusted operating income in the segment was $912 million versus $314 million a year earlier. The medical benefit ratio fell to 91.3% from 92.8%. Medical membership was 26.1 million as of September 30, remaining consistent with June 30.

Health Services revenue was $54.0 billion, up 9.5%, primarily from pharmacy drug mix and brand inflation, partially offset by pharmacy client price improvements. Adjusted operating income was $2.11 billion, up 2.7%, from improved purchasing economics and drug mix. Pharmacy claims processed were 481.7 million, up 1.3% on a 30-day equivalent basis.

Pharmacy & Consumer Wellness revenue was $35.5 billion, down 2.0%, from regulatory-related price reductions, recent generic introductions and pharmacy reimbursement pressure, partially offset by drug mix, prescription volume including Rite Aid asset acquisitions completed in the third quarter of 2025, and brand inflation. Adjusted operating income was $1.64 billion, up 10.6%, from core pharmacy strength and Rite Aid contributions, partially offset by business investments and consumer dynamics. Prescriptions filled were 470.0 million, up 1.9% on a 30-day equivalent basis, from utilization and Rite Aid prescription files, partially offset by the absence of Omnicare long-term care volume after the September 2025 deconsolidation.

In Corporate/Other, the company recorded a $193 million Omnicare contingent guarantee liability in the third quarter after bankruptcy court approval of litigation and Chapter 11 agreements and a probable fourth-quarter 2026 sale of Omnicare assets.

More than 69 percent of Aetna Medicare Advantage members are in 2027 plans rated 4 stars or higher, the company said, positioning Aetna second among large, publicly traded Medicare Advantage organizations in the Star Ratings program.
