Vertex Raises 2026 Revenue Outlook After Crinetics Close

The biotech lifted full-year revenue guidance to $13.15 billion to $13.25 billion after third-quarter sales of $3.44 billion.

Vertex Pharmaceuticals Incorporated (VRTX) raised its full-year 2026 revenue outlook to $13.15 billion to $13.25 billion after third-quarter sales of $3.44 billion and the September 1 close of its Crinetics Pharmaceuticals acquisition.

The global biotechnology company said the lift from the prior $13.1 billion to $13.2 billion range embeds continued cystic fibrosis growth, $500 million or more from CASGEVY and JOURNAVX combined, and $40 million to $50 million from PALSONIFY after the deal closed. Guidance still includes about 150 basis points of foreign-exchange benefit.

Total revenue rose 12% from the third quarter of 2025, matching the year-over-year pace of the second quarter, when sales were $3.33 billion. Chief Executive Officer Reshma Kewalramani said results were marked by growth across a more diversified portfolio, pipeline progress, and the Crinetics close.

U.S. revenue increased 10% to $2.17 billion, a slower year-over-year rate than the 11% U.S. rise in the prior quarter. The company attributed the U.S. increase to CF demand, including new ALYFTREK initiations, higher realized CF net prices, and contributions from CASGEVY, JOURNAVX, and PALSONIFY. Outside the U.S., revenue rose 16% to $1.27 billion, faster than the 14% year-over-year increase in the second quarter, from CF performance, ALYFTREK uptake, more CASGEVY infusions, and a favorable foreign-exchange impact.

CF product revenue was $3.27 billion. ALYFTREK contributed $706 million, TRIKAFTA/KAFTRIO $2.44 billion, and other CF products $131 million. CASGEVY revenue was $99 million, up 30% from the second quarter and nearly six times the $17 million a year earlier. JOURNAVX revenue was $58 million, up 18% sequentially and nearly tripling the $20 million a year earlier; more than 750,000 prescriptions were filled, more than 40% quarter-over-quarter growth. PALSONIFY added $10 million in September after the Crinetics close.

GAAP net income fell to $654 million, or $2.56 a share, from $1.1 billion, or $4.20 a share, a year earlier. The company said the decline was primarily due to Crinetics-related expenses. Non-GAAP net income was $1.2 billion in both third quarters. Combined GAAP R&D, acquired IPR&D, and SG&A expenses were $2.2 billion versus $1.5 billion a year earlier; non-GAAP combined expenses were $1.5 billion versus $1.3 billion, including about $500 million of Crinetics compensation and transaction-related costs. The company said the increases also reflected commercial investment for a renal franchise led by povetacicept in IgAN and the JOURNAVX launch.

Vertex increased full-year combined GAAP R&D, AIPR&D, and SG&A expense guidance to $7.15 billion to $7.2 billion from $6.3 billion to $6.45 billion, and raised non-GAAP combined expense guidance to approximately $6.0 billion from $5.65 billion to $5.75 billion, to reflect Crinetics operations and integration for the rest of 2026. GAAP guidance includes about $500 million of those compensation and transaction costs.

Cash, cash equivalents, and marketable securities were $9.8 billion as of September 30, 2026, down from $12.3 billion at year-end 2025, primarily due to $9.0 billion of net cash used to acquire Crinetics and share repurchases, partially offset by a $4.5 billion term loan.

The company said it established a fourth commercial pillar in rare endocrine diseases with the Crinetics close and is preparing a fifth in renal medicine. The BLA for accelerated approval of povetacicept in IgAN is under FDA priority review, with a PDUFA date of November 30, 2026. If approved, povetacicept would be the first commercialized therapy in Vertex’s emerging nephrology franchise.
