Insmed Lifts Brensocatib and Amikacin 2026 Sales Floors

Third-quarter revenue reached $524.4 million as BRINSUPRI posted $400.1 million and the company raised full-year ranges for both products.

Insmed Incorporated (INSM), a global biopharmaceutical company, reported third-quarter 2026 revenues of $524.4 million, up from $425.5 million in the second quarter, and raised the low end of its full-year 2026 sales outlook for both commercial products.

BRINSUPRI (brensocatib) generated $400.1 million in the third quarter, a 29% increase from the second quarter after 49% sequential growth in the second quarter versus the first. The company attributed the third-quarter lift to strong growth in the United States. U.S. BRINSUPRI sales were $398.1 million, compared with $28.1 million a year earlier; international BRINSUPRI revenue was $2.0 million.

Chair and Chief Executive Officer Will Lewis said the U.S. launch “continues to exceed expectations” and that the results supported raising the low end of full-year 2026 revenue guidance for both products. Insmed lifted 2026 BRINSUPRI guidance to $1.35 billion to $1.4 billion from $1.25 billion to $1.4 billion.

ARIKAYCE (amikacin liposome inhalation suspension) revenue was $124.4 million, up 9% from the third quarter of 2025 after 8% year-over-year growth in the second quarter. The company said the increase was primarily driven by international markets. U.S. ARIKAYCE sales were $75.5 million, up 2% from $74.0 million a year earlier; international ARIKAYCE revenue was $48.9 million, up 21% from $40.3 million. Insmed raised 2026 ARIKAYCE guidance to $460 million to $470 million from $450 million to $470 million.

Net loss narrowed to $100.3 million, or $0.46 a share, from $370.0 million, or $1.75 a share, a year earlier. Operating loss was $89.4 million.

Cost of product revenues excluding amortization was $78.2 million, versus $29.4 million a year earlier. The company said the increase primarily reflected higher product revenues and that the cost ratio declined because BRINSUPRI has lower manufacturing costs than ARIKAYCE. Research and development expenses were $270.0 million, compared with $186.4 million a year earlier, which the company tied to manufacturing costs, TPIP clinical work, and headcount. Selling, general and administrative expenses were $253.0 million, versus $186.4 million a year earlier, which the company attributed primarily to professional fees and compensation tied to BRINSUPRI commercial activity.

In August 2026, Japan’s Ministry of Health, Labour and Welfare approved BRINSUPRI Tablets 25 mg for non-cystic fibrosis bronchiectasis in adults and pediatric patients 12 years and older. Insmed launched the product in Japan in October 2026, describing it as the first and only MHLW-approved treatment for the condition.

In September 2026, the FDA accepted a supplemental new drug application for ARIKAYCE in Mycobacterium avium complex lung disease as part of a combination regimen, granted Priority Review, and set a PDUFA target action date of January 28, 2027. Insmed said it plans to submit Phase 3 ENCORE data to Japan’s PMDA in the fourth quarter of 2026 to support potential label expansion to all patients with MAC lung disease.

Treprostinil palmitil inhalation powder programs advanced. The company said it had enrolled more than 35% of patients in Phase 3 PALM-ILD in pulmonary hypertension associated with interstitial lung disease and expects to complete enrollment in the first half of 2028. It enrolled the first patient in Phase 3 PALM-PAH in July 2026 and initiated Phase 3 PALM-PPF in September 2026. Insmed continues to anticipate starting a Phase 3 TPIP study in idiopathic pulmonary fibrosis in the first half of 2027.

Cash, cash equivalents, and marketable securities totaled approximately $1.1 billion as of September 30, 2026. The company continues to anticipate peak revenues of more than $7 billion for BRINSUPRI, more than $6 billion for TPIP, and more than $1 billion for ARIKAYCE.
